Fundación Valentia Huesca

Conecta Rural is a project promoted by Fundación Valentia Huesca, selected in the Fundación Mapfre +Rural 2025 Grant Program, and co-financed by the European Social Fund Plus (ESF+). Its goal is to improve or maintain the functionality and personal autonomy of people living in rural areas through a model of specialized local care.

The project is being developed in the Monegros region, in the Grañén health district, and is aimed at people with intellectual disabilities, neurodegenerative diseases, secondary effects of stroke, or other pathologies with a functional impact. Conecta Rural offers personalized care through a unique integrated service that includes functional assessment, individualized intervention plans, and specialized professional support, provided either at home or in nearby community settings. The project seeks to guarantee effective access to specialized care, reduce regional inequalities, and encourage people to remain in familiar surroundings, strengthening coordination with social and health services in the region.
